Church name and positions and length of service you and your spouse have held in the church prior to starting deputation.
Do you faithfully support the missions program of your church financially? Do your pastor and church sanction your missionary work?
When did you surrender to the mission field? When did you gain assurance of where the Lord wanted you to go?
When did you start deputation? What percent of your support have you raised? How long do you anticipate your deputation will take?
Define your burden and vision. In other words what do you see your mission work to be? As of now, give a brief overview of what you anticipate the first five years on the mission field to be? ..i.e. What will be the first order of business? What activities do you anticipate will consume your time? What will be your primary focus? Give an overview of where you hope to be in 10 years. Be specific.
What involvement has your pastor held in your ministry since you surrendered to the mission field? Does your pastor feel you are ready to go to the mission field? If no, give details. Has he suggested additional training, schooling, support, or the need for more maturity? If so give details.
What is your philosophy regarding evangelism? Define repentance and conviction as well as defining their role in salvation. Is there one group of Independent Baptists that would best identify your philosophy of evangelism? Define “easy believism”.
Give your position regarding the King James 1611 Bible? Do you believe the King James 1611 Bible is error free? Do you believe there are passages that could be translated more correctly?

Males wear pants, and females wear dresses most of the time. When immodesty becomes an issue with certain activities, then there can be exceptions.
The Bible only dictates modest, moderate, and gender specific dress, and anything other than this is legalism

What Bible will you use on the field? What is it’s underlying Greek text? If it is not the Received Text, is there a Bible in that language that has been translated from the Received Text?
